(Seguin) — Rural veterans in Guadalupe County will soon have greater access to vital health services right in their own homes. Information on the newly expanded Home & Community Based Care program for rural veterans will be among the list of resources made available at today’s Veterans Informational Fair. The event will be held today from 10 a.m. to 2 p.m. in the Veterans Outreach Center (VOC) located at 205 E. Weinert Street.

Hosted by Guadalupe County Veterans Services, the free event is open to the public and is an opportunity for veterans and their families to connect directly with VA representatives.

Organizers say staff from various VA programs including Homeless Services, Women Veterans Program, Caregiver Support, Community Care, and the National Cemetery Administration will be on-site to answer questions and assist with enrollment and referrals.

One of the key resources being made available at today’s event will be the Home-Based Primary Care program, part of the VA’s broader Home & Community Based Care offerings. Veteran officials say the VOC now hosts a Home-Based Primary Care PACT (Patient Aligned Care Team), strategically located in Seguin.

This program brings a team of medical professionals directly to the homes of eligible veterans who have difficulty traveling due to physical or mental health conditions.

To qualify, veterans must be enrolled in VA healthcare. They must also have challenges leaving home for care and they must be referred by their VA primary care provider.

“Our veterans have earned the right to accessible, compassionate care, no matter where they live,” said Nancy Russell, Guadalupe County veteran services director. “We are excited to have this additional service in our county. It will make a life-changing difference for rural, homebound veterans who have long faced barriers to receiving care.”
